• onset age of the disease
• family history
• recent HbA1c reading
• date of recent HbA1c reading
• type of treatment
• height and weight
• general health

Once we have this vital information we can then get to work and find the best premiums for your situation.

Once you have applied for cover the insurer will ask for a diabetes questionnaire to be completed. They may also ask for further information from your diabetes consultant or GP.
